Important Dates for Planning 2018 Form Submission
E-Mail the completed 2018 Allocation Form — in Excel format — and all supporting documents and reports to Mike Missios at email@example.com no later than April 1, 2017.
Mail the signed form and accompanying financial documents to the Chancellor’s office to be received at the Metropolis no later than April 1, 2017. The allocation form must be accompanied with the 2016 Income Statements (profit and Loss) and Balance Sheets. Without these documents, your form submission will not be considered complete.
A late penalty of up to 10% will be applied for allocation forms and supporting documentation received after May 1, 2017.
